91 Million Lives
One American agency you were rarely asked to think about prevented more than 91 million deaths over two decades. Sit with a number larger than every death in World War II, counted in the column of lives saved. Trump, his administration and Musk, tore that agency down in a matter of weeks last year. The man who held the chainsaw says zero people have died. The peer reviewed science says something his lawyers would rather you never read. Here is what happened, who decided it, and the toll already landing on real families right now.
The 21 Years of Data Should Stop You Cold
In July 2025, researchers from the United States, Spain, Brazil, and Mozambique published a study in The Lancet, one of the oldest and most respected medical journals on earth. They looked at 21 years of data across 133 countries. Their finding: USAID funded programs prevented more than 91 million deaths between 2001 and 2021, including over 30 million children, and high levels of funding lined up with a 65 percent drop in deaths from HIV and AIDS, a 51 percent drop from malaria, and a 50 percent drop from neglected tropical diseases.
This was the machinery of survival in motion. Antiretroviral drugs flowing through PEPFAR, the American AIDS relief program, to more than 20 million people with HIV. Mosquito nets and antimalarials moving through supply chains across Africa. Food for starving children in Sudan, Somalia, and refugee camps along the Myanmar border. For about 17 cents per American per day, around 64 dollars a year, this agency made the United States the largest funder of global health on the planet, at roughly a third of one percent of all federal spending.
How It Was Taken Apart
The dismantling began days after Trump returned to the White House in January 2025. Elon Musk, the billionaire running the new Department of Government Efficiency, pointed at USAID and called it a locus of corruption and waste. By his own description the agency made up about half a percent of government spending. He waved a chainsaw on stage as the mascot of the cuts.
Within weeks, the administration permanently cancelled 83 percent of USAID’s global contracts, roughly 7.7 billion dollars in active programs. Secretary of State Marco Rubio took over in February and folded the few survivors into the State Department. On July 1, 2025, USAID formally closed its doors after more than six decades. The cuts sliced overall American aid funding roughly in half, from about 68 billion dollars to 32 billion. Former Presidents George W. Bush and Barack Obama joined former staffers and the singer Bono on a video call to mark the closure, with Obama calling it a travesty and a tragedy.
The Denials Are On The Record
Here is where the people responsible planted their flag. When Musk left DOGE at the end of May 2025, he wrote on X that zero people have died. When Senator Jeff Merkley pressed Rubio about the rising death estimates at a congressional hearing that same month, Rubio was firm: no one had died, he called the modeling exaggerated, and he argued there was no total freeze. When the newest study landed this February, a senior State Department official dismissed The Lancet as a failed journal and called the research rooted in outdated thinking.
Hold those three statements in your mind. Zero deaths. No one has died. A failed journal. Now look at what the evidence shows.
What The Evidence Shows
The same July 2025 study ran the projection forward. If the cuts hold, the models point to more than 14 million additional deaths by 2030, including over 4.5 million children under the age of five, around 700,000 dead children every year. The lead researcher, Davide Rasella of the Barcelona Institute for Global Health, put it plainly: for many poor countries, the shock would match a global pandemic or a major war.
A companion study in The Lancet Global Health arrived this February, one year after the shutdown, widening the lens to count the cascade as other nations followed America’s lead. Its central projection lands at 9.4 million additional deaths by 2030, roughly 2.5 million of them children under five, and its severe scenario reaches 22.6 million deaths, including 5.4 million young children.
Real deaths are already being counted, in real time. Brooke Nichols, an infectious disease modeler at Boston University, built a public dashboard called the Impact Counter, and by late June 2025 her model estimated more than 332,000 people had already died because of the cuts, including over 224,000 children. Winnie Byanyima, head of the United Nations HIV program, projected an additional 6.6 million HIV infections over four years and 4.2 million more AIDS related deaths, 300,000 of them children. The Center for Global Development, working outside the Lancet research, estimated 500,000 to 1,000,000 lives lost in 2025 alone.
A Honest Accounting
Strong claims deserve honest accounting, so here is the fair version. Every one of these figures comes from a statistical model. These are projections built on spending records and disease data. No one has walked the world counting bodies in a morgue. The numbers shift with assumptions about how fast services collapse and how long the gap runs between losing treatment and dying. The House Foreign Affairs Committee sent Nichols a letter attacking her math, arguing she divides total appropriated funding by average treatment cost and treats potential reach as real impact. Even careful voices with no stake in the studies land in the same place.
Lee Crawfurd of the Center for Global Development put the honest version in one breath. Treat the precise numbers with caution. The overall conclusion is likely correct. People will die in large numbers. Serious experts argue over the size of the catastrophe. They agree on its direction.
The Fight Happening Right Now
This stopped being abstract over the past few days. On the podcast I’ve Had It, Congressman Ro Khanna of California said Elon Musk possibly sentenced 4.5 million children to death by taking apart USAID, and he called for Musk to be subpoenaed and investigated. Khanna tied the accountability to one condition, a Democratic takeover of Congress this fall. The 4.5 million figure comes straight out of the July 2025 Lancet study.
Musk, fresh off becoming the world’s first trillionaire after the SpaceX public offering, answered on June 22 with one line: time to sue this liar. He defended DOGE’s process, saying the standard was simple, aid recipients had to hand over contact information to verify the money reached real people, and he pointed to a Justice Department bribery case involving a former USAID official, calling Khanna a robber who belongs in prison. Khanna answered by challenging Musk to a public debate and refusing to back down.
Watch the move closely. A man worth more than a trillion dollars threatened a lawsuit against an elected official for quoting a peer reviewed study out loud. The reflex of the wealthy, when the body count gets attached to their name, is to reach for a lawyer.
Where The Money Went Instead
One more detail turns this from tragedy into something colder. As of this May, the administration moved to pull 2 billion dollars Congress had set aside for global health, money marked for malaria, tuberculosis, maternal and child health, nutrition, and HIV and AIDS, and steer it toward the cost of shutting USAID down, the legal fees and unpaid invoices and asset sales. Analysts estimated the redirection alone puts roughly 121,000 people on track to die from tuberculosis and at least 47,600 from malaria, severs nutrition aid for nearly 23 million young children, and ends safe childbirth support for over 5.7 million women.
Read that again. They pulled lifesaving money away from dying children and spent it on the paperwork of ending the program built to keep them alive. Seventeen Senate Democrats called the maneuver an illegal impoundment, part of 3.2 billion dollars in expiring funds the administration moved to spend on the shutdown, after Congress had explicitly refused to give it legal authority to close the agency.
What This Means For You
You did not sign off on this. The money came out of your taxes, 64 dollars a year, and it bought something most Americans say they want, children living through malaria and AIDS. They made the decision fast, a handful of people, with a chainsaw for a mascot and a rounding error for an excuse.

Call your representatives and ask one direct question: where do they stand on restoring global health funding.
